Before we begin

This questionnaire prepares us both for the most productive audit possible. Your answers let me come in already oriented so we’re not starting from scratch. There are no right answers here. The most useful responses are honest ones, even when they reveal gaps.

Questions marked “optional” are completely optional. If you have the data and are comfortable sharing it, it will sharpen the audit. If not, skip them and we’ll work with what you have.

What happens next

Once we receive your completed questionnaire, we’ll review it before our kick-off call and come prepared with any follow-up questions. We’ll also begin mystery shopping your program during Week 1, so if there’s anything we should know about your enrollment or top-tier experience before we do that, note it above or reach out directly.

The goal of this questionnaire isn’t to put you on the spot. It’s to make sure the few weeks we spend together are spent on the real problems, not discovering the obvious ones.
